The members of the Wellness Team seek your help in identifying Barton students who may be in need of guidance, support, and/or intervention. The purpose of this report is for you to be able to alert Wellness Team members to students whose actions and behaviors cause concern. This report will not be used to initiate disciplinary action, but rather to get help for the student.

Reporting Sexual Assault
This form can be used by members of the Barton Community College community (including guests) who have experienced or witnessed sexual violence (sexual assault, rape, or sexual battery). The information allows Barton to gather important data about our campus climate, identify any troubling trends needing attention, and develop policies, prevention strategies, and programming that can help ensure a safe, healthy, and nondiscriminatory campus environment.
If no information is provided regarding the identity of the victim, no further action will be taken upon receipt of this information.
If identifying information about the victim is provided, a confidential advocate will contact the victim to conduct follow up. However, information shared in this form alone will not result in a formal investigation by way of Barton’s established grievance process. All individuals are encouraged to file a formal report with Barton Campus Safety Office, Barton’s Title IX Coordinator (Cathie Oshiro, (620) 792-9234, firstname.lastname@example.org), or one of Barton’s Deputy Coordinators (Angie Maddy, (620) 792-9226, email@example.com or Julie Knoblich, (620) 792-9275, firstname.lastname@example.org).
In compliance with the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act (Clery Act) (20 USC 1092(f)), information submitted via this form reflecting a reportable crime as defined by the act, will be included in required crime log reporting. No names are included as part of the report.
